What families should know before services begin

These answers explain referrals, authorizations, service areas, and caregiver options. Our office can clarify next steps for your situation.

  • We provide non-medical Personal Care, Homemaking, In-Home Respite, and Community Living Supports. Services are delivered according to each member’s authorized care plan and may include help with bathing, dressing, grooming, mobility, meals, light housekeeping, safety, routines, and caregiver relief.

  • We match qualified caregivers to authorized needs, provide orientation and ongoing support, and follow the member’s approved care plan. Our office communicates with clients, families, care managers, and health plans as appropriate to address changes, scheduling concerns, and service quality.

  • Submit the request-care form or call our office at 248-716-4818. We will collect the information needed to review the referral. Medicaid and MI Choice services must be authorized by the member’s health plan, care manager, or waiver program before covered care can begin.

  • We serve Wayne, Oakland, Macomb, Washtenaw, Livingston, and Monroe counties. Availability depends on the member’s location, authorized services, and caregiver coverage.

  • We accept referrals and authorizations from Molina Healthcare, Wellcare by Meridian, and The Senior Alliance through the MI Choice Waiver Program, subject to current contracting, eligibility, and service authorization. Contact our office so we can review the member’s plan and referral status.

  • Some Medicaid or MI Choice participants may be permitted to select a qualified family member as a paid caregiver. The health plan, care manager, or waiver agency determines eligibility, approved services, caregiver qualifications, and authorized hours. Our office can explain referral and employment steps after authorization.
